Step 1 — Submit ANNUAL SERVICE REPORT (ASR) by April 30
All credentialed individuals in The Wesleyan Church are required to submit Annual Service Reports. Failure to do so can mean forfeiture of credentials. The deadline for your ASR to be submitted is April 30. Those who do not file by that date will be reported to the Annual District Conference as Unsatisfactory and further action may be taken. Please do your part and submit your report in a timely manner.
MINISTERIAL STUDENTS / LICENSED MINISTERS: Annual Ministerial Student & Licensed Minister Reporting (click) is on its own page. If you are a District Pastor, please return to this page for instructions on submitting all other required Local Church Reports by May 31.
Last year most credentialed individuals submitted their ASR the quick and easy way, via the online Wesleyan Pastor Portal (click), and this method is now required by HQ and the Kentucky-Tennessee District.

He will be glad to answer your questions and walk you through the entire process of submitting your ASR via the Pastor Portal. Unless you are a Pastor, once you have submitted your ASR, you are done! Congratulations!

Pastors, follow Steps 2 to 7 below to submit all other required Local Church Reports by May 31.
Step 2 — Submit LOCAL CHURCH STATISTICAL REPORT by May 31
Statistical information ( from May 1 through April 30 ) must be entered via the Pastor Portal (click). You may wish to download and print a hard copy of the following Local Church Statistical Report (LCSR) for a working copy as you gather information to be entered online. This information may be entered by either the Pastor or an approved Church Admin who has been granted access to the Pastor Portal.
If you encounter difficulty submitting statistical information via the Pastor Portal, after downloading and printing a working copy of the Local Church Statistical Report (LCSR) form (below) and filling it in completely, please contact Office Admin Tim Linick | firstname.lastname@example.org | 270-790-1341 to have him input statistics for your church directly into the Pastor Portal. Please do not submit a HARD COPY of the LCSR. This info must be submitted via the Pastor Portal.

Step 3 — Submit LOCAL CHURCH LEADERSHIP INFO by May 31
Following your Annual Local Church Conference and Elections, you are required to update your local church leaders within the Pastor Portal (click). This information may be updated by either the Pastor or an approved Church Admin who has been granted access to the Pastor Portal.
The following Elected Officers must be updated each year:
- LBA Secretary
- Church Treasurer
- Lay Delegates
- Alternate Lay Delegates
It is also helpful if you can update various other leaders such as:
- Sunday School Superintendent
- Director of Youth Ministry
- Director of Children's Ministry
- Church Trustees
- LBA Members-At-Large
If you have difficulty updating leadership info via the Pastor Portal, please contact Office Admin Tim Linick | email@example.com | 270-790-1341 to have him update your leaders directly into the Pastor Portal.

Step 4 — Submit Nominee(s) for NEHEMIAH AWARD by May 31

firstname.lastname@example.org | of any local members/attendees that you would like to see honored as a recipient of a "Nehemiah Award for Excellence in Lay Ministry" (see criteria below).
Criteria: Attendees of local churches who exhibit the following characteristics:
- They care enough about the local church and ministry to get involved and do what they can to meet the challenges of the day.
- They keep their focus on God.
- They refuse to give up and keep laboring even through frustrations, discouragement, and increasing obstacles.
- They are balanced in their desire to build the church and defend against the enemy and his desire to destroy.
In your email, please GIVE A BRIEF DESCRIPTION of how your nominee has been a Nehemiah in your church.
Step 5 — Submit LIST OF MEMBERS PROMOTED TO HEAVEN by May 31

email@example.com | of any local members who have passed away over the District year (from May 1 to April 30). Please include the following info:
- Member's Full Name
- Date of Birth
- Date of Passing
- Link to Online Obituary (if possible)
Step 6 — Submit HARD COPY of the Following to DS by May 31:
- Annual Pastor's Report — This is the full report of your past year's activities as pastor. This should have been given at your Annual Local Church Conference and should also include vision casting for the year ahead.
- Local Church Treasurer's Financial Report — Include all local church accounts (Savings & Checking) in format of Income/Expense Report from May 1 to April 30
- Local Church Membership Roll — Current List as of May 1
- Current Property Insurance Declarations Pages — For all Buildings / Vehicles Owned by the Local Church

Step 7 — Submit ANNUAL REPORTS CHECKLIST by May 31
Scroll back through all six steps above and make sure that you have completed each and every step. Submit the following ANNUAL REPORTS CHECKLIST (click) by May 31. When you are certain you have completed each of the first six steps and the ANNUAL REPORTS CHECKLIST has been submitted, you are finished!
